A/R Charge Card Form
Instructions on how to fill out the request for temporary PMU white A/R charge card form for student organizations and campus departments.
Student Organizations
- Download form
- Fill in the name of the event or group
- Fill in the date(s) of the event. If it is one day, please write the same date in both the “From” and “To” sections. If the event is more than one day please write the appropriate days in the “From” and “To” sections. ***Please note that we require at least one week from the activation date to process a card. If we are not allowed at least one week from the activation date of the card, we may not be able to process the card. In this case, the PMU food units do accept other forms of payment such as: cash, check, and credit cards.***
- Please fill in the dollar limitations for breakfast, lunch, and supper. If there are no limitations please put “no limit” next to the appropriate meal. If you do not need the card for breakfast, then leave that line blank. This also works for lunch and supper. It is the responsibility of the individual requesting the Temporary PMU White A/R Charge Card to ensure that the food unit is open for the event as some unit’s business hours may vary.
- Circle the location(s) where you would like to use the card. Please include an estimate of how many individuals will be at the event. This helps the PMU to have our cashiers staffed appropriately.
- Please circle “deliver the cards to the retail locations if you want the cards to be waiting for at the register. Circle “other” if you want the cards mailed to a campus building or picked up at the PMU BO. Please explain which you would prefer.
- Circle “Badge” if your group will be handing out badges for the individuals attending the event. If so, please send a copy of an original badge with the request form for the A/R Card. Circle “Verbal” if you will not be using a form of identification. We do not recommend this method as anyone can claim to be part of the event.
- Please put fill in the student organization’s official name. Please do not abbreviate.
- If you know your PMU A/R number, please fill it in. Otherwise write your BOSO number here.
- Fill in the contact name and a phone number where you can be reached. The person requesting the form must sign along with the President and Treasurer or Advisor of the Student Organization. The person requesting the card must obtain a signature from BOSO. (Please note that you may have other forms to fill out from BOSO.)
Campus Departments

- Fill in the name of the event or group

If we are not allowed at least one week from the activation date of the card, we may not be able to process the card. If you do not need the card for a particular meal, then leave that line blank. This section does not apply to the Rec Center, unless you note otherwise. - If you want the cards to be waiting for you at the cash registers of the units which were circled earlier in #4, please circle “deliver the cards to the retail locations.” Circle “other” if you want the cards mailed to a campus building or picked up at the PMU BO. Please explain which you would prefer.
- Our cashiers need to know when they are authorized to use the white AR card that was delivered to them. Please indicate by circling which method of identification you would like our cashiers to use. If you select “Badge” or “Ticket,” an original copy of the badge or ticket needs to be included with this request form. Circle “Verbal” if you will not be using a form of identification. We do NOT recommend the “Verbal” method, as anyone can claim to be part of your event.
- Please fill in the department name. Do not abbreviate.
- Please fill in your University Fund and Cost Center.
- If you know your PMU A/R number, please fill it in. (Hint: most PMU A/R numbers are # 8 followed by the home department number. An example would be 81280 for Engineering Admin).
- Fill in the contact name and a phone number where you can be reached. The person requesting the form must sign along with someone representing the requestor’s Business Office. This allows the requestor’s Business Office to expect charges on a future statement from PMU.
